Community Outreach Lead

Our Healthy Hearts Together community outreach lead will work with local residents to boost heart health and overall wellbeing! You will develop the Healthy Hearts Together project in partnership with West Wakefield and Trinity Primary Care Networks (PCN’s), reporting to the Wellbeing Service Manager. Your role is to support health equity by identifying people at greater risk of Cardiovascular Disease and promoting healthy lifestyles. You will provide a non-medicalised approach to providing information, support and activities to a diverse range of people, targeting those in greatest need by tailoring the ‘Five ways to wellbeing’ model to meet their needs.

Contract: 1-year fixed term contract with potential for extension

Pay: C3 £20,700 - £23,700 pro rata

Hours: 32 hours per week

Early Years Practitioner – Qualified

To support the delivery of high-quality nursery provision for babies, young children and their families in accordance with current guidance and legislation. Building relationships with small groups of children and their parents/carers as a Key Worker.

Contract: Permanent

Pay: Salary to be Discussed

Hours: 37 Hours Per Week

Relief Early Years Practitioners (Qualified or Unqualified)
