Community Outreach Lead, Healthy Hearts Together



Role at a glance:

The Healthy Hearts Together Project will be delivered by St George’s Lupset with West Wakefield and Trinity Health Group Primary Care Networks (PCNs).

This project prioritises physical and mental wellbeing in an integrated way focusing on the relationship between physical cardiovascular health and the overall health and wellbeing of the population. You will be working with other partners to provide information, support and activities, using a non-medicalised approach.  You will also work with individuals and groups in the community to achieve positive wellbeing outcomes.



We’re looking for someone who:

  • has a desire to effect positive change,
  • the ability to connect effectively with a range of people locally,
  • can identify opportunities and develop activities to meet identified needs and wants of local people,
  • has a good knowledge of the VCS,
  • is passionate about supporting people,
  • is motivated to make a difference,
  • has excellent communication and IT skills with knowledge of Microsoft Office,
  • can be forward thinking with monitoring and evaluating using an outcomes-based approach.

Contract: 1-year fixed term contract with potential for extension

Pay: C3 £20,700 - £23,700 pro rata

Hours: 32 hours per week
